Quick answer

To become a speech-language therapist in Malaysia, SPM leavers normally complete an accepted science-based pre-university qualification before entering a recognised four-year bachelor’s degree in speech science, speech pathology or speech-language pathology. Speech-language therapist is a regulated allied-health profession, so the exact degree, clinical education, Malaysian Allied Health Professions Council registration and practising-certificate requirements must all be checked.

2. Can you study speech therapy directly after SPM?

Normally, you do not enter professional practice through an SPM-entry speech-therapy diploma. The established Malaysian qualifications are bachelor’s degrees such as Bachelor of Speech Science with Honours, Bachelor of Speech Pathology with Honours and Bachelor of Speech-Language Pathology (Honours). MQR records show four-year, MQF Level 6 structures.

SPM leavers therefore usually complete Foundation in Science, STPM, Matriculation, A-Level, IB or another accepted qualification first. A related diploma can be considered by some universities, but it is not the same as a direct professional Diploma in Speech Therapy.

3. What speech-language therapists actually do

Speech-language therapists assess and manage communication and swallowing difficulties across the lifespan. Their work can involve speech-sound disorders, delayed language, stuttering, voice, social communication, acquired language disorders after stroke or brain injury, motor-speech disorders and feeding or swallowing difficulties.

Assessment may include interviews, observation, standardised tests, language samples, oral-motor examination and collaboration with families or other professionals. Intervention is individualised and can involve direct therapy, caregiver training, communication strategies, environmental changes and alternative or augmentative communication.

The profession requires patient listening, precise observation, clear documentation and comfort working with children and adults who may communicate differently. Observe more than one setting before deciding whether the work fits you.

4. Route 1: Foundation in Science

Foundation in Science can be a focused route when it contains the Biology, Chemistry, Physics or Mathematics components required by your target degree. It can also preserve alternatives in audiology, occupational therapy, physiotherapy and other health sciences.

The key risk is portability. One university’s internal foundation may not be accepted by every speech-language therapy programme, and progressing to the degree may require results well above the foundation’s general pass mark.

  • Which exact speech science or speech pathology degrees accept the foundation?
  • Which science and Mathematics modules are compulsory?
  • What CGPA and individual subject grades are required?
  • Is MUET or another English result required?
  • Is an aptitude test or interview part of selection?
  • Which alternative degrees accept the completed foundation?

5. Route 2: STPM and Matriculation

STPM and Matriculation are important routes to public-university speech-language therapy programmes. The current requirements vary by institution. UKM, for example, publishes specific academic, science-subject, English and interview conditions for its speech-science degree, while USM publishes a separate combination for speech pathology.

Use the current UPU e-Panduan for your applicant category. Check general university conditions, special programme requirements, interview arrangements and merit calculation.

Meeting a minimum CGPA only establishes eligibility. Programmes have limited clinical-training capacity, so selection can be competitive.

6. Route 3: A-Level, IB, AUSMAT and SAM

A-Level and other recognised pre-university qualifications can support applications where the exact university accepts them. Science requirements are not identical across programmes. USM currently lists routes involving two subjects from Biology, Chemistry and Physics or Mathematics, while UKM publishes its own subject and grade conditions.

  • Does the university accept the exact qualification and examination board?
  • How are grades or aggregate scores calculated?
  • Which sciences must be taken, and at what level?
  • What English-language evidence is required?
  • Is the interview compulsory for every applicant category?
  • Does the completed degree support Malaysian registration?

Obtain written confirmation when a published requirement does not clearly cover your certificate.

8. Science subjects and English matter

Speech-language therapy combines health science, linguistics, psychology and clinical practice. Biology supports understanding of development, neurology and body systems; Chemistry and Physics contribute scientific foundations; Mathematics supports data interpretation and research. Programme requirements determine the exact combination.

English proficiency is important because many programmes use English-language textbooks, assessments and clinical terminology. Therapists also need strong Bahasa Malaysia and culturally responsive communication when working with Malaysian clients and families. Additional languages can be valuable, but fluency alone does not replace clinical training.

Check MUET, IELTS, TOEFL or other accepted evidence for the exact programme and intake.

9. Prepare for the programme interview

UKM and USM publish interview requirements for their speech-related degrees. The interview may help the faculty assess communication, motivation, awareness of the profession and readiness for a clinically demanding course. It is not a performance test for already knowing therapy techniques.

  • Explain the difference between speech-language therapy and nearby professions.
  • Describe what you learned from observing or researching real clinical settings.
  • Discuss child and adult practice rather than focusing on pronunciation alone.
  • Show that you understand communication and swallowing are both within the field.
  • Use clear, respectful language without making promises about outcomes.
  • Prepare examples of teamwork, persistence, listening and ethical judgment.

Answer honestly. A thoughtful explanation of why the work suits you is stronger than memorising technical terms.

10. How to verify a speech-therapy degree

  1. Search the MQR. Match the full award title, institution, campus, MQF level, study mode and accreditation dates.
  2. Check the MAHPC information. Review the current recognised-qualification masterlist and registration guidance.
  3. Confirm your cohort. Ask whether the accreditation and professional-recognition position cover your expected study period.
  4. Verify clinical education. Request placement settings, supervision, required hours and competency assessment.
  5. Ask about registration. Obtain the institution’s written explanation of how graduates register under Act 774.
  6. Save the evidence. Keep dated copies of official records and written responses.

11. What you study in a speech-language therapy degree

AreaExamples
Foundational sciencesAnatomy, physiology, neurology, psychology and human development
Speech and languagePhonetics, linguistics, language development, acoustics and speech science
Clinical disordersSpeech sound, language, fluency, voice, motor speech and cognitive communication
Feeding and swallowingTypical swallowing, dysphagia assessment, safety and management principles
Professional practiceEthics, counselling, documentation, teamwork and evidence-based practice
Clinical educationSupervised assessment and intervention with children and adults

Curricula differ. Ask how bilingual and multilingual assessment, augmentative communication, swallowing, simulation and interprofessional learning are covered.

12. Clinical placements are central

Clinical education develops the ability to gather a case history, select appropriate assessments, interpret findings, plan intervention, communicate with families and document care safely. A programme needs suitable clients, qualified supervisors and facilities—not only classroom teaching.

  • Which university clinics, hospitals, schools or community services are used?
  • Are child, adult, communication and swallowing experiences included?
  • How many clinical hours or competencies must be completed?
  • Who supervises students and how is competence assessed?
  • Are placements guaranteed and arranged by the university?
  • What are the travel, accommodation, health-screening and repeat-placement costs?

13. Registration and practising certificates under Act 774

Speech-language therapist is listed among the allied-health professions regulated by the Allied Health Professions Act 2016. The Malaysian Allied Health Professions Council manages the applicable recognition, registration and practising-certificate processes.

The Malaysian Health Care Practitioners’ System states that practitioners practising in Malaysia must be registered with the relevant council and hold the applicable practising certificate. Completing a degree and obtaining legal permission to practise are separate milestones.

  1. Complete the recognised qualification and required clinical education.
  2. Prepare the documents required for MAHPC registration.
  3. Apply through the current MHPS process.
  4. Obtain the applicable practising certificate before practice.
  5. Maintain current professional and renewal obligations.

14. Career settings and the public-service route

Speech-language therapists may work in public and private hospitals, university hospitals, rehabilitation centres, early-intervention services, special-education settings, community programmes, aged care, universities and private practice. Work opportunities depend on registration, vacancies, experience and employer requirements.

SPA currently includes an honours bachelor’s degree in speech, audiology or a related field under Pegawai Pemulihan Perubatan Gred U9. Its role description states that the speech officer provides rehabilitation for communication, fluency and voice difficulties and counselling services. These are appointment criteria, not a guarantee of a government vacancy.

Later professional development can focus on paediatrics, adult neurological communication, swallowing, voice, fluency, augmentative communication, education, research or service management.

16. Calculate the complete pathway cost

Pre-university tuition + degree tuition + compulsory fees + clinical equipment + health checks and vaccinations + placement travel or accommodation + living costs + possible repeat costs − confirmed aid = estimated pathway cost

Compare the complete five-year or longer journey, not only the first year. A cheaper foundation can become costly if it is not accepted by the intended degree, and distant clinical placements can add significant transport or accommodation expenses.
